On a personal note, I understand your frustration at having to fill out all the paperwork we are sending to you. It is however very important that you fill it all out as accurately as possible. Much of the paperwork has to do with insurance information and personal history as well as the history of your current complaint. This information helps me to determine the best course of action to take on your behalf. And after all it is much better to fill the paperwork out at your leisure rather than here in the office.
